Contractor Procedures for Obtaining Access to DMB-managed facilities:

• Access the DMB Contractor Clearance Form (DMB-453) below.  Review the instructions and complete the form.
• Submit the completed form to the DMB Customer Service Center via:
  • e-mail to DMB-Customerservice@michigan.gov
  • U.S. Mail to:  DMB Customer Service Center, John Hannah Building, First Floor, 608 W. Allegan Street, Lansing, Michigan 48909
• The approved DMB Contractor Clearance Form will be valid for 1 year from the date of receipt unless otherwise indicated.
• The contractor is responsible for updating and maintaining the form.
